Assistherhood Training Agreement & Waiver of Liability

1. Acknowledgment of Risks
I, the undersigned parent or legal guardian, understand that participation in basketball training, clinics, and related activities involves inherent risks, including but not limited to physical exertion, contact with other participants, use of sports equipment, and movement on gym floors or outdoor courts. I acknowledge that these activities may result in injury, illness, or other medical issues.

2. Assumption of Risk
I knowingly and voluntarily assume all risks, whether known or unknown, associated with my child’s participation in Assistherhood events, including but not limited to accidents, falls, collisions, overexertion, and equipment-related injuries.

3. Medical Clearance
I certify that my child is physically fit to participate in basketball activities and has no medical condition that would prevent safe participation. I agree to inform Assistherhood staff of any relevant health concerns prior to participation.

4. Release and Waiver
In consideration for my child’s participation, I release, waive, and hold harmless Assistherhood, its directors, officers, coaches, volunteers, and agents from any and all claims, demands, causes of action, or liability arising out of or related to any injury, illness, or other damages sustained during participation, except in cases of gross negligence or willful misconduct.

5. Emergency Authorization
I authorize Assistherhood staff to provide first aid and to arrange emergency transportation to the nearest medical facility if necessary. I agree to be financially responsible for any medical treatment or services provided.

6. Media Release (Optional)
I grant permission for Assistherhood to photograph, video record, and use my child’s likeness for promotional, educational, or informational purposes in any media, without compensation.

7. Acknowledgment of Understanding
I have read this Training Agreement & Waiver of Liability in full, understand its terms, and agree to be bound by it. I understand that by signing, I am waiving certain legal rights.
